From New York State Thruway

Take the thruway to Exit 18/New Paltz. Turn right on Route 299. Proceed to the end (Intersection 9W). Turn right on 9W South. The office is approximately ¼ mile on the right.

From Poughkeepsie

Take Route 9 to the Mid Hudson Bridge. Bear right, following the signs to Route 9W North (Highland). Take 9W north (approximately 2 miles) to the traffic light (299 Intersection). Turn right into the Park and Ride. Make a "U" turn in the Park and Ride turning left on Route 9W heading south. The office is ¼ mile on the right.

From Rhinecliff Bridge

Proceed over Kingston/Rhinecliff Bridge. Take first exit after bridge, Route 32. At stop sign, make a left onto Route 32 South. Proceed 2.6 miles. At third traffic light, make a left onto 9W ( Koenig Blvd.). Proceed 1 mile. Take first exit, Delaware Avenue. At traffic light, make a right onto Hasbrouck Avenue. Proceed 0.8 miles to end of road. At stop sign, make a right onto Grand Street. Second building on left, Mid-town Medical Arts Center, 43 Grand Street. Parking in both front and rear of building.

From N.Y. State Thruway

From the Exit 19 Traffic Circle, take "Kingston-Broadway"Exit, Route 587 (also called Colonel Chandler Dr.) to the first traffic light. Go through light crossing over Albany Ave., and bear left onto Broadway. Go approximately 1 mile. You will see a YMCA on the right. Stay in left hand lane and turn left onto Dederick Ave. Go straight through to the parking lot in the back of the building, Mid-Town Medical Arts Center. We are the first suite on the left, 43 Grand Street. If you pass Dederick Ave., make a left at the light, which is Grand Street. Go to middle of the block. We are on the left. You can park in the lot on the right. If you come in this entrance, walk straight through to the end. We are the last suite on the right.

From 32 South

Right onto Greenkill Avenue. At the blinking light, make a left onto Clinton Avenue. Take a right at the first road. Go up to Broadway and make a right at the light. Go past the underpass, staying in the left lane. Make a left onto Dederick Ave. Go straight through to the parking lot in the back of the building. We are the first suite on the left. If you pass Dederick Ave, make a left at the light, which is Grand Street. Go to middle of block. We are on the left, Mid-town Medical Arts Center. You can park in the lot on the right. If you come in this entrance, walk straight through to the end. We are the last suite on the right.

From Mid-Hudson Bridge/9W

From Mid-Hudson Bridge, take 9W North approximately 15 miles to the Delaware Ave. Exit. Take Delaware Ave. exit all the way to end ( approximately 3 miles). This becomes Hasbrouck Avenue. Proceed 0.8 miles to end of road. At stop sign, make a right onto Grand Street. Second building on left, Mid-Town Medical Arts Building, 43 Grand Street. Parking in both front and rear of building.

For your convenience, we participate in a large number of health insurance plans. Please bring your insurance cards with you at the time of your visit. Also, many insurance companies and HMO's require the patient to obtain a written referral from a Primary Physician. Each patient is responsible for obtaining the proper referral from his/her doctor. You may bring referrals with you at the time of your visit or have them faxed directly to our office at (845) 691-7589. If you don't see your insurance company listed, please call the office as changes and updates to our accepted insurance plans are made periodically.
